Adding conditional formatting to forms will allow you to have certain fields appear on a form if a certain answer is selected for a question.
You can set up conditional formatting for the specific question you would like to appear based on the form submitter’s answer from another question.
To do this, please find below some instructions.
1) On the forms module, select the form you would like to edit. You can edit the form by selecting the icon to the right of the form with the 3 icons.
2) On the question you would like to apply the conditional formatting to, click the pencil icon to edit the question field.
3) A popup will appear. Click the Advanced tab and check mark the box next to the Enable Conditional Logic section.
4) Select the conditional formatting for the question. Please see below some notes to consider for the conditional formatting.
The area will allow you to show or hide a question field if a certain value is selected.
This Field If Section
This section will allow you to have the question field appear or be hidden depending on the values you enter below. For example, by selecting all, all values entered below would need to match in order for the question field to appear or be hidden. If Any is selected, the question field will appear as long as any value entered below matches.
Values - Section 1
Select the question field that will prompt the selected question field you are editing to appear or be hidden for the form submitter.
Values - Section 2
Indicate how you want the field to appear depending on the value the form submitter chooses. For example, in the screenshot below, we have chosen “is” so the question field that we are editing will appear if the answer related to the question indicated on the left drop down box is a selected answer.
Note: The “is” filter is best used for question fields with predefined answers like radio buttons, dropdown etc. If your question includes a text box field where answer can vary, you can use other filter options such as “contains”.
Values - Section 3
Select the answer value that would prompt the question field you are editing for conditional formatting to appear or be hidden.
To add more conditional formatting values select the + button to the right of the value. To delete, click the trash can button.
